PARTNERS CONNECTED HEALTH (PCH)—a part of Partners Healthcare—is studying and implementing new ways of incorporating digital and mobile technologies into patient care. This year PCH launched a new program across the Partners system, which includes dozens of hospitals and clinics, that lets patients collect health data such as blood pressure at home using a variety of digital and wearable devices. The devices transmit the data via Bluetooth to patients’ smartphones, where an app then sends the information to Partners’ electronic health record system. The aim is to give doctors a better picture of their patients’ health, and for patients to feel more engaged in their care.
